Needs and Concerns of Home Carers of Elderly with Dementia |
The impact of caring for elderly with dementia is a growing concern as it is projected by 2030, approximately 24,000 people will be affected with this disease (Ministry of Health 2001). This presentation will report on the findings of a two-staged quantitative and qualitative exploratory study which aims to identify the needs and concerns and nature of stressors experienced by a group of female homes cares. A 10-items PSS test was administered to 21 female home carers, 15 responded. 5 were selected for further exploration through non-standardised interview. Major themes emerged were – trajectory of disease, carers' feelings, coping resources and future concerns. |

Perception of Mental Health Nursing by Foreign nurses in a psychiatric hospital. |
A descriptive study on 63 foreign nurses on their perception of mental health nursing, the problem encountered, and the stressors experienced while working in a local psychiatric hospital, using questionnaires. Results showed that there was no negative perception of mental health nursing by the foreign nurses. |

The Effects of Exercise on the Functional Status of the Elderly Persons Living in the Long Term Care Setting of a Psychiatric Hospital. |
The purpose of this study was to explore the relationship of exercise and its effects on the functional status of the elderly persons living in the long-term care setting of a psychiatric hospital. A simple 20-minute exercise program was introduced to the elderly persons living in the long-stay wards of Woodbridge Hospital for 3 months. Their mobility and functional status were taken pre and post the exercise program using the Elderly Mobility Scale and the Modified Barthel Index respectively. Results showed that the simple 20-minute program does improve the functional status of the elderly persons staying in the long-term care setting. |

Compliance Therapy in psychotic patients: A pilot study on an intervention to improve inpatients' attitude to treatment compliance. |
The study examined the effectiveness of compliance therapy on 69 subjects with relapsed schizophrenia or depression using motivational interviewing techniques. Results showed that patients in individual or group session had improved attitude scores after the intervention. |

A Descriptive Study On The Needs of Caregivers In Response To Their Newly Diagnosed Relative. |
A descriptive study on the needs of caregivers to their newly diagnosed relatives was conducted in IMH with a total of 64 respondents. This study aimed to identify the needs of caregivers and to heighten the practitioner's awareness so that future interventions and support systems can be developed to help caregivers deal with their pain and burden. The results have highlighted the family's strong need for psycho-education programme, support groups, family consultation and their involvement in planning of care. |

Close monitoring of discharged patients in the community by Community Psychiatric Nurses helped in reduction of re-hospitalisation. |
A retrospective quantitative study of 123 schizophrenic patients, with Community Psychiatric Nurse's monitoring period between eight months to one year, discharged from CPNS in 1998 were recruited. Interventions were obtained from the patients' hospital case-notes to determine the effectiveness of the CPNS in reducing the readmission rate of these patients before and during follow-up monitoring by the CPNS nurses. The survey showed that 29.3% had no readmissions. The mean length of stay for CPN-intervention admissions was shorter compared to before intervention admissions with no differences between discharges during and after CPN intervention. |

Attitudes and Perceptions of Female Nurses Towards Working in Male Forensic Ward |
A descriptive study to explore the causes perceived as unsafe to work at the male forensic wards by female nurses of a local psychiatric hospital using a self-administered questionnaires. A total of 292 questionnaires were distributed to female nurses working in a state psychiatric hospital. The response rate was 80.1% (n= 234). Findings indicated that most female nurses would not mind to work in a male forensic ward if they received better benefits. The possible exposure of sexual harassment played a significant factor in influencing their decision to work in a male forensic ward. |

Impact of verbal abuse by patients on staff working in the psychiatric wards |
A descriptive study to examine and describe the contents of verbal abuse and the impact they had on nurses. Included in the study were all grades of nurses, both local and foreign in the short-stay wards of Woodbridge Hospital . Data collection was done through a self-administered questionnaire on aggressive incidences experienced by the nurses over 1-month period. Results showed 64% of the nurses experienced an immediate negative emotion to the incidence, however after a positive approach to it, 47.6% came out of the situation unaffected |

A retrospective descriptive study on the first 307 patients on the Relapsed Schizophrenic Pathway from July to Sept 2003 was done using a survey form to analyse whether the pathway had helped to enhance the nursing practice. Results showed that the pathway did support nursing practice by the provision of knowledge, development and planning of programs and clinical activities, and provision of quality care to the patients. |

Health Educational Needs of Patients with Acute Schizophrenia: Caregiver's Perceptions |
The aim of this study was to explore the caregivers' perceptions of the educational needs of the patients with acute schizophrenia. A non-experimental, descriptive/exploratory survey study using questionnaires was used. It was conducted in the outpatient clinic of a large, local psychiatric hospital. The results revealed that the most important educational needs of the acute schizophrenic patients perceived by the caregivers were: ‘information on how to handle side effects' (99.3%), ‘ways for solving problems'( 99.3%), ‘ways of managing stress more effectively' ( 99.3%), ‘to manage anger, violence and assaultive behaviour( 99.3%)', and ‘to know more about mental health services' ( 99.3%). |

A Comparison Study on Nurses and Therapist' Perception on the Positioning of Stroke Patients in SGH |
Background: Presently, there are various position strategies for stroke patients, but these are primarily based on clinical experience rather than research results. Before developing a study to evaluate the effect of positioning on outcome after stroke, it was important to explore whether there were differences in perception between Neurology/Neurosurgery nurses, nurses from other wards, Occupational Therapists and Physiotherapists over the positioning of stroke patients in SGH.
Methods: Questionnaires were sent to 227 nurses, 7 Physiotherapists and 6 Occupational Therapists from 6 wards where more stroke patients are cared for in SGH.
Results: 72% of the respondents identified bed positioning as its' positioning strategy. 58% believed that preventing complications was the most important aim. In additions, the therapists were more likely to rank “Encourage functional recovery of affected limb” as the most important aim than the Ward 52B & 74 nurses. 45% believed that the best position for conscious hemiplegic stroke patients was “sitting in a chair”. There was no significant difference on selecting these positions as best position between nurses and therapists.“A 30-degree propped-up angle in bed” and “lying horizontally on the unaffected side” were selected as the best positions for nursing unconscious hemiplegic stroke patients. There was less of a consensus between nurses and therapists as to whether it was appropriate for unconscious stroke patients to lie horizontally on the unaffected side or 30° propped-up angles in bed.62% also believed that they do not rely on physiotherapists to communicate positioning strategies for their stroke patients. However, 89% pointed out the lifting and transfer technique taught by therapist are useful.
Conclusions: The lack of consensus between these respondents is probably because there are few researches to guide nursing practices for the positioning of stroke patients. Hence, research to confirm which positions improves or hinders outcome after stroke is indispensable. Indeed, positioning is a simple inexpensive strategy, which could have a substantial public health impact, as stroke is so common. |

Nurses' Perceptions Towards Caring for Dying Patients in Oncology Ward and General Surgery Ward |
Aim: The aim of the study was to explore the attitudes of nurses towards dying patients in the Medical Oncology and General Surgical wards.
Methods: A descriptive study was used to explore the relationship among the demographic variables and nurses' attitudes towards the dying patients. Issues on communication between nurses and patients, education in nursing dying patients, and support are also explored. A structured questionnaire consists of socio-demographic data, views on nursing dying patients, ethical issues and communication/education was utilized. 122 Registered nurses and Enrolled nurses from both wards were recruited. Data was analysed using SPSS 11.5. The associations between demographic variables and the various level of nurses' personal emotions were assessed using chi-square/Fisher's exact test.
Results: Nurses who work in ward 48 were 3.97 (95% CI 1.47 to 10.75) times more likely to agree that taking care of dying patients reminded them of their own mortality (p = 0.007). Nurses in ward 58 were 4.49 (95% CI 1.53 to 13.16) times more likely to agree that they are frightened of dying patients (p=0.006). Registered nurses were 3.18 (95% CI 1.12 to 9.02) times more likely to agree that it was depressing caring for dying patients (p=0.048). Nurses whose patients have uncontrolled pain were 12.82 (95% CI 1.50 to 111.1) times more likely to agree they feel helpless towards dying patients (p=0.006) but were 6.58 (95% CI 1.24 to 34.48) times likely to agree that caring for dying patients makes them treasure life more (p=0.043).
Conclusion: The findings of this study showed that future in-house training plans could be initiated to provide a greater understanding about the care for dying patients. |

A survey study to determine the incidences of back pain experienced by nurses |
Aim: The aim of the study is to assess whether the use of assistive devices reduces the incidences of nurses experiencing low back pain. The findings of this study will help to determine the need to purchase assistive devices for lifting/transferring patients.
Methods: Survey questionnaires were distributed to nurses from Ward 64. The findings were analysed using Statistical Package for the Social Sciences (SPSS) version 11.5. Association between categorical variables was assessed using Chi-square or Fisher's exact tests. Logistic regression model was used to compare nurses' low back pain (LBP) status between using assistive devices or not.
Results: A Fisher's exact test results showed that there were no significant association between use of assistive devices during lifting/transferring of patients and getting LBP (p>0.05).
Discussion: Majority of nurses attributed their low back pain (LBP) to lifting/transferring of patients. Most of them used assistive devices during lifting/transferring of patients. The unavailability of assistive devices was identified as the main reason for not using assistive devices. There were no significant associations between compliance to proper lifting technique and getting LBP. Problems with lifting technique, lack of manpower, unable to wait for assistance and unavailability of assistive devices were cited as reasons for not always complying with proper lifting technique. Weight of patient was identified as the significant factor for experiencing LBP. Conclusion: Results did not support that the use of available assistive devices reduces the incidences of nurses experiencing LBP. There is a need to explore further on the availability and accessibility of assistive devices used for lifting/transferring patients. |

A Perception Study on Job Posting and Job Satisfaction of Newly Graduated Nurses Assigned to the Specialised Inpatient Wards in SGH |
Aims: To investigate the perception of newly graduated nurses based on the relationship of job posting and job satisfaction.
Methods: This was a quantitative cross sectional study involving newly graduated female nurses. The inclusion criteria was nurses with ages ranging from 20-25 years, who had graduated with a Diploma in Nursing, registered with the Nursing Board and started their nursing career either on16 June 2003 or 01 July 2003 respectively. A self administered structured questionnaire was used to collect data on the relationship of job posting and satisfaction from nurses who worked in the specialised wards for a period of 6 months after graduation. Convenience sampling method was used to enrol all 45 participants who joined the institution for the study.
Results: A total of 37 completely filled questionnaires were received from the participants giving a response rate of 82%. Results were analysed using descriptive statistics. There was a statistically significant link between the participants' job satisfaction and posting to their area of choice at the end of the six months period from the point of starting their career (significance level set at 0.05). Details of the study results will be presented.
Conclusions: This study shows that there is a definite effect of job posting on job satisfaction level in newly graduated nurses. Although this sample size is small, it is reflective of the % of graduands posted to the institution during the period for the study. A longitudinal study working with other institutions to obtain a bigger sample size would provide further confirmation of the findings and call for intervention to improve on the nurses job posting. This would have an impact on the recruitment and retention of nurses. |

Job Satisfaction of Nursing Staff Providing Patient Education |
Patient satisfaction has become an important indicator to measure the quality of nursing care. The purpose of the study is to explore the relationship between nurse staffing level and patients' satisfaction of quality nursing care in an acute hospital setting based on nurses' perspectives. This is a correlational study involving 467 participants by self-administered questionnaires. Proportional stratified sample was used to select the participants (SSN & SN: SEN & EN: HCA=310:101:56). Measurement of the subject's include demographic data, opinion questions and total satisfaction score. Patients satisfaction of quality nursing care from nurses perspective measured by total satisfaction score was significantly related to nurse staffing level (p=0.059), designation (p<0.05), age (p<0.05), department (p=0.009), class status (p=0.023), qualification (p<0.05) and years of experience (p=0.015). Controlling all the co-variables in order to find out the real significance between nurse staffing level and patients' satisfaction of quality nursing care needs further study. |

Patient-Reported Quality of Life and Sexual Function Following Radical Retropubic Prostatectomy for Prostate Cancer Through a Third Party Interview |
Aim: To determine the patient's health-related quality of life (HR-QOL) and sexual function following Radical Retropubic Prostatectomy (RRP).
Methods: This is a retrospective and descriptive study involving patients who had RRP done from the year 2001 to 2002. Subjects were identified through the department's prostate cancer database. Interviews were conducted through an independent third party. To measure HR-QOL, questionnaires made up of 47 questions about physical, functional, social/ family well-being, emotional, relationship with doctor and additional concerns related to prostate cancer issues were utilised. For screening of sexual function, the 5-item International Index of Erectile Function was used as tools.
Results: A total of 26 patients were interviewed. Their mean age was 65.5 (range 55-83). 96.2% were Chinese and 3.8% Malay. The overall HR-QOL was not impaired after surgery. The functional well-being and erection domains were the typical quality of life that decreased after surgery. Using Fisher's exact test (p < 0.05), significant difference were seen in the functional well-being aspect of the HR-QOL domains for those older than 65 years. For the sexual function, 46.2% of men reported that they had no sexual activities after surgery. The proportion of men who were able to have erections was higher in those younger than 65 years (33.3%) compared with the older groups (14.3%).
Conclusion: The overall results of HR-QOL were not affected but the sexual quality of life was diminished by surgery. Information obtained can assist the nurse practitioner in providing accurate preoperative counselling and patient teaching. This can also ultimately enhances client care towards a healthy postoperative rehabilitation. |

A Descriptive Study on Quality of Life of Haemodialysis Patients |
Aim: The purpose of this study was to explore the perceived QOL in haemodialysis patients and identify the factors that may affect QOL.
Methods: A convenience sample of 26 outpatients with ESRD were selected.
Results: The overall perceived QOL (mean 60.7, SD 13.6) were relatively average. There was no relationship between demographic variables with QOL, although females had lower QOL score. Most patients viewed kidney disease as a burden to them and the families.About 50% were upset with the diet and fluid restrictions
Conclusions: This study provides useful information for nurses in caring for ESRD patients. Identifying suitable coping skills may result in better adaptation leading to better QOL. |

A comparative study on prevention of pressure sores in high risk elderly patients with or without application of barrier cream in hospital |
Aim: This study aimed to determine the efficacy of barrier cream in preventing pressure sores.
Methods: The study was conducted on 120 elderly patients of SGH over 60 years of age. Sixty of them were from the experimental group, whereby barrier cream was applied to their gluteal/sacral region in conjunction with two hourly turning and the usage of pressure relieving mattress. The other 60 patients were from the control group, whereby only two hourly turning and pressure-relieving mattress were provided.
Results: The results seem to suggest that application of barrier cream help to prevent pressure sore formation. However there is much less focus on the effectiveness, therefore it is essential for further research to be carried on the evaluation of 2 hourly turning, use of pressure relieving mattress and other risk factors such as nutrition and mobility.
Discussion: It is found that subjects who were bedfast is 2.64 times (95% CI 7.99 to 9.34, p=0.036) more at risk as compared to those who were chairfast. Any effective strategy of preventing pressure sore development would require the inclusion of risk assessment and the resources needed to counter the risk.
Conclusion: Provision of application of barrier cream will not eliminate the need for nurses to check patient's pressure areas. Appropriate care must still be given. Patients require turning for many reasons. The application of barrier cream combined with nurses' education on risk factors and preventive measures should commence prior to discharge thus reduce the length of hospitalization. |

Discussion: Among all the variables, qualification did not have an impact on the total satisfaction score. However, years of experience, class status, department, and age greatly affect the total satisfactory score. High designation staff has a lower satisfactory score. It could be explained that majority of the lower designation nurses employed from overseas hold at least a bachelor degree in nursing in their own country.
Conclusion: Patients' satisfaction of quality nursing care from nurses' perspective was significantly related to nurse staffing level, but there were other co-variables which are more significant. Controlling all the co-variables in order to find the real significance between nurse staffing level and patients' satisfaction of quality nursing care needs further study. |

A Descriptive Study on the Views of Patients and Nurses Concerning Visitation Policy in a Hospital |
Aims: To identify patient's and nurses' perceptions towards visitation policy in SGH. The findings will be useful in planning hospital visitation policy which will benefit patients' well-being, increase customer satisfaction and enhance service delivery in health care.
Methods: This is a quantitative descriptive design using self-developed questionnaire. A total of 260 participants that involve 120 registered nurses and 140 patients in SGH. Questionnaires were distributed to the ward SNM to assist in disseminating questionnaires to registered nurses due to poor response from 5 sessions conducted in WD 74 tutorial room. As for in-patients, the researchers approached and explained to the participants personally before the surveys. All participants were chosen based on selection criteria.
Results: 83.3% of nurses and 47.2% of patients prefer our present visitation policy in our hospital. There are large differences in the percentage of nurses and patients who chose open visitation. 65.6% of patients and 20% of nurses prefer open visitation. Chi-squares are used to find any correlations between nurses and patients who have different views on the statements. In patients, we found that age and education has an influence on the statement ‘helps patient to recover faster in the presence of family members'.
Discussion: The majority of nurses prefer restricted visitation but most of the patients prefer open visitation. Most of the nurses who choose our present visitation policy actually rank providing quality care to patient as the most important factor. Coincidentally nurses who choose open visitation also thinks that workload is the most important factor while most of the nurses choose restricted visitation rank workload as the second important factor.
Conclusion: Results support the literature that nurses often think that workload is the most important factor that affects them. It also supports the perception that nurses traditionally prefer restricted visitation while patients prefer open visitation. |

Evaluation of Key-hole Taping with indwelling Urinary Catheter on the Lower Abdomen |
Aim: The purpose of this study was to evaluate the effectiveness of the key-hole taping method on patients with indwelling catheter on the lower abdomen.
Methods: A randomized control design was used for this study. A total of 30 patients was sampled, which yielded a power of 90%. The subjects were randomized into two groups. The control group (n = 15) received conventional taping method. The study group (n = 15) received the key-hole taping method. Approval for the study was obtained from the SGH Ethics Committee.
Results: There was a significant difference in the effectiveness between key-hole taping method and the conventional taping method (p < 0.001). All subjects who used the key-hole taping method did not experience any retaping after the first one. On average, patients who used the key-hole taping method were about twice as comfortable compared to those who used conventional taping method after adjusting for sex, the number of times of walking, contact with water, and getting out of bed (p=0.001).
Conclusions: This study showed that there was statistically significant difference between the key-hole taping and the conventional taping methods. The key-hole taping method enabled the catheter to be stabilized more securely in ambulant patients with indwelling urinary catheter on the lower abdomen. The key-hole taping has the advantage of avoiding the catheter being dropped off easily. Hence, this may allow the patient to rest more comfortably and not being unduly disturbed. |

A Study on Perceived Work-Related Stressors by the Newly Recruited Foreign and Locally Graduated Registered Nurses |
Aim: The purpose of this study was to identify the perceived stressors and examine the expectations of the newly hired foreign registered nurses and newly graduated local registered nurses.
Methods: A quantitative descriptive survey design was used to survey 125 registered nurses. The sample comprised 52 newly joined foreign registered nurses from Philippines and China , and 56 local registered nurses. A 25-question questionnaire with a 4-point Likert scale was used to collect data.
Results: Local registered nurses had more difficulty in applying theory in practice (c2=8.61, df=1, p=0.003) and felt that they lacked experience in clinical skills (c2=8.33, df=1, p=0.003). In addition, they felt that they were inadequate in handling patient's/relative's enquiries as compared to foreign-hired registered nurses (c2=3.62, df=1, p=0.043). Although local nurses felt ‘burn out' by the end of the shift (c2=7.62, df=1, p=0.005), they still felt enthusiastic to come to work (c2=5.19, df=1, p=0.018). Both foreign and local registered nurses felt that doctors did not view them as competent (c2=7.62, df=1, p=0.005). 92.8% of local and foreign registered nurses indicated that they had preceptors to guide them, with almost half of them (41.2%) having 2-3 months long of preceptorship. 89.1% of nurses felt that the perceptorship programme had been useful.
Conclusions: In contrary to what was expected by the researchers, local nurse felt more stressed than foreign nurses. This was attributed by lack of clinical and working experience in the hospital. Utilising foreign nurses as the benchmark, it would appear that more clinical experience in different environments might help local nurses cope better with stress. Perceptorship programmes had proved to be helpful for both local and foreign nurses in the initial period of role transition as it provides them with a pillar of support and guidance. |

A Study on the use of Inhaled Corticosteroids by patients in SingHealth Polyclinics |
Aims: To assess patients' knowledge and to determine patients' attitude and practice towards the use of inhaled corticosteroids in SingHealth Polyclinics.
Methods: This is a cross sectional descriptive study. Subjects were selected through convenience sampling of adults who sought acute and continuing care for asthma. A total of 416 subjects with history of bronchial asthma and who were prescribed with inhaled corticosteroids were interviewed from September 2002 to August 2003.
Results: The mean age of the 416 subjects was 41.2 (SD 10.3), out of which 51.4% are males. 71.9% of the subjects had history of asthma for more than 10 years and 45.7% reported family history of asthma. Most of them (81.2%) used their inhaled steroids as prescribed but 58.9% reduced their dosages when they felt better. More than 30% of the subjects who were bothered by asthma morbidities were found to be in the moderate and severe persistent groups. 21.2% of the subjects were fearful of weight gain and 21.2% had the misconception that it would cause addiction.
Conclusions: Although majority of the subjects adhered to the use of inhaled steroids but there were
still some misconceptions about inhaled steroids. Reinforcing educational programmes, organising
asthma support group sessions would be useful in allaying fear and anxiety and to increase adherence. |

A One Year Survey of Cervical Cancer Screening in a Primary Care Centre in Singapore |
Aims: To study the quality of papanicolou (PAP) smear done by the doctors and nurses and the incidence of cervical cancer in a typical primary health care centre.
Methods: This is a retrospective study involving 1381 women from March 2002 to February 2003. 77.3% of the subjects (age: 30 to 49years) and 18.5% of the subjects (age: 50 & above) and 4.2 % of the subjects (age: 20-29years) were enrolled. All smears taken were sent to Singapore General Hospital Cystology for evaluation.
Results: Doctors performed 57.6% of Pap smear and the remaining 42.4% were done by nurses. The results showed that there is no difference in detection rate for negative for malignancy, infective and abnormal smears by the doctors and the nurses. There is also no difference in percentage of inadequate smears. 8% of the women underwent the test for the first time, 9.4% within the past one year, 52.6 of the women had Pap smear done 1 year or more ago and 30% more than 2 years ago.
Discusion: The study showed that more than half of the women had a pap smear test done within 1 to 2 years as recommended by the National Cervical Cancer Screening programme. 99% of the smear
taken were found to be adequate and satisfactory. 1.5% of the smears taken were found to have
Squamous cells abnormality. Only one woman is detected/confirmed as cervical cancer.
Conclusions: The findings suggested that the smear specimens taken by trained registered nurses
and doctors are satisfactory and of high quality. Nurses are equally competent in the technique in
performing Pap Smear. |

Routine Screening of MRSA for Elective CABG Surgery is not Necessary: A Retrospective Study |
This retrospective study aimed to assess MRSA carrier state in a cohort of cardiothoracic patients admitted for elective CABG and to determine the correlation of MRSA colonisation with surgical site infection. 974 subjects were recruited. 82.5 percent (803) were tested negative for MRSA colonisation. 30 days post-surgery review shown that 3.8 percent (37) of the patients had surgical site infection and 96.2 percent (937) had no infections. 1.3 percent (13) had MRSA infections and 2.5 percent (24) were infected with other organisms. The correlation of MRSA colonisation with surgical site infection is not statistically significant (p=0.506). It is also not clinically significant as this study revealed that patients who were MRSA carrier did not influence the infection rate (Chi-square = 0.088).Patients had their surgery regardless of the MRSA colonisation status. Therefore, the study suggest that routine screening for MRSA for elective CABG is not required. |

Emergency Nurses' Perception of
Discharge Advice for the Eldery Patients
with Falls. |
This descriptive quantitative study was undertaken to investigate the nurses' perceptions on giving discharge advice and to determine strategies that can help to improve discharge advice for the elderly in ED. Questionnaires were distributed to a convenience sample of 80 ED nurses from 2 local restructured hospitals. 91.3% (n=73) respondents maintained it was important to give discharge advice to elderly patients with falls. Family members played an important role when discharge advice was given. Inadequate time and language barrier were the main contributing factors affecting ED nurses in giving effective discharge advice. Although a majority (89%, n=72) agreed that they have adequate knowledge and skills and were confident in giving advice, they till found it stressful. 95.9% (n=77) highlighted useful strategies like pamphlets and discharge checklist as useful strategies |

A Descriptive Study on
Registered Nurses' Level of
Knowledge on Deep Vein Thrombosis |
Background: Deep vein thrombosis (DVT) is potentially a life-threatening condition. With prompt assessment and appropriate prophylactic regimen DVT could be prevented. Study aims to evaluate registered nurses (RNs) knowledge level on DVT.
Methods: 159 RNs participated in the study through purposive sampling. Questionnaires on participants' work experience, educational level and specialties area were collected. Pearson's correlation tests were used to analyze the relationships between
these variables.
Results: There was a significant relationship between longer years of work experience and the RNs' level of DVT knowledge (p = 0.001). However, the relationships of RNs' level of knowledge between educational levels and area of specialized and non-specialized trained RNs' were found to be insignificant
(p = 0.375 and p = 0.803 respectively).
Conclusion: DVT education and prevention programmes for RNs' to be developed further in the hospitals. This will ensure early detection and prevention measures of DVT patient
management.. |

Parkinson's disease (PD) is a chronic degenerative disorder that demands much care from caregivers. Nursing care for patients with PD is an enormous task and lack of knowledge of the disease can lead to poor delivery of care to patients. The aim of this quantitative descriptive study is to gain an insight into the registered nurses' level of knowledge on PD and its nursing care. A convenience sample of 198 registered nurses was selected for this study. Self-reported questionnaires were the research instrument. Findings showed that the overall categorical percentage of scores for questions answered correctly were 60.3% for General Knowledge of PD, 69.2% for Nursing Care, 43.2% for Nutritional matter, 65.8% for Activities of Daily Living and 33.8% for Medication. |

A Descriptive Study on Registered Nurses' Attitudes toward Death and Dying Issues in Patients with Cancer and their Families |
This study investigated the possible relationships amongst the demographic variables of registered nurses and their attitudes towards death and dying issues in caring for patients with cancer and their families. A non-experimental descriptive survey design was employed to study a convenience sample of 120 registered nurses from cancer wards from 2 restructured hospitals. The instrument is a self-designed questionnaire. Findings revealed that the amount of contact with dying patients, religion, year of experience in nursing and ethnicity were related to nurses' attitudes towards caring for the dying patients. |

Paediatric Nurses' perception of the health education needs of asthmatic patients aged between 5-7 years of age |
This study aims to explore the perceptions
of registered nurses on the health education needs of
caregivers of children aged between 5-7 years old. It is
a non-experimental, descriptive study design. The 6 areas
of health education on compliance, triggers, advice, inhalation
techniques, knowledge on asthma and history-taking were
surveyed, using a 4-point Likert scale questionnaire consisting
of twenty-four items and1 open-ended question. The study was
conducted on 108 registered nurses in a restructured hospital.
The survey showed that 99.1% of registered nurses perceived
compliance and advice as important and most important,
whereas 100% perceived triggers, inhalation technique,
and knowledge on asthma as important and most important.
93.5% perceived history-taking as important and most important.
Years of service and nursing qualifications do not have
significant influence on their perceptions.
The results of the survey showed that all 6 areas of health
education are important for the effective management of
asthma and should be included in the teaching of caregivers.
Educating nurses in these areas would empower them to
render more effective health teaching. |

Caring Behaviour as perceived by women in labour |
This study aims to identify the 10 most important caring
behaviors perceived by the women in labor and to determine
if there are any differences in perceptions between the primiparas and the multiparas. It is a descriptive study. Convenience samples of 62 women who have had normal vaginal deliveries were asked to complete a structured questionnaire comprising a modified Care-Q Instrument. Results showed behaviors such as “checks on patients frequently”, “gives patients' treatments and medications on time”, “responds quickly to patients' calls” and “provides encouragement” were among the 10 most important perceived caring behaviors. Participants felt that it was very important for the midwives to be easily accessible to them, to be able to monitor their conditions closely and carefully. It was also essential for midwives to understand their individual needs.
The study showed no difference in perception of caring behaviours between the primiparas and multiparas. |

Perception of perioperative nurses on role expansion |
The purpose of this study is to explore how much
perioperative nurses know of their role in the
upcoming advancement of their job scope, to evaluate
how well perioperative nurses understand their roles and
acceptance of psychological aspect of the new challenges
ahead. A stratified sample of 50 perioperative nurses
both trained and non-trained in a local General Hospital ,
major operating theatre was surveyed. Closed-ended
survey questionnaires were used for data collection. Findings
showed that perioperative nurses have strong desire for
role expansion as it signifies new challenges and empowerment
towards quality care of patients. |

A Descriptive Study on Academic
Stress and Coping Strategies
Amongst Second and Third Year

This study presents the findings of a quantitative, descriptive
study on the academic stress and coping strategies amongst
Year two and Year three nursing students. A convenience
sample of 253 Year two and 234 Year three students participated
in the study. A self-designed structured questionnaire comprising
the modified Student Stress Survey and Life Stress Inventory
were the research tools. It was found that the stress score of
the students was moderately high with examinations being
the most prevalent. However, students perceived their coping
strategies such as having adequate sleep, having some
entertainment eg. going to the movies as healthy and effective. |

Urinary Incontinence in Women: Prevalence and Factors Influencing their Health Behaviours |
Abstract
Urinary Incontinence (UI) is a distressing and debilitating condition which affects one's quality of life. It is highly prevalent and burdensome condition among women of all ages.
Aims of study: The purpose of this research is to investigate the prevalence of UI and its different types. In addition, the factors influencing their health behaviours were also being explored.
Design: This descriptive, quantitative, non-experimental study was conducted through face-to-face interview using a researchers-created questionnaire. Pilot study was done to determine the clarity and readability of the questions. Samples were obtained from women (aged 16 and above) in the general community. The exclusion criteria include individual with hearing impairment, on urinary catheter and those with language barriers. A convenience, non-probability sampling method of data collection was conducted over duration of two weeks. Compiled data were analyzed using SPSSWIN.
Results: Of the women surveyed, 28% (n=84) reported involuntary loss of urine. Of these, 81% has Stress UI. Two-third of women with UI symptoms has vaginal childbirth. Alarmingly, 95.2% (n=80) did not seek treatment for this problem. Also, educational level influenced to whom women prefer to discuss UI with.
Conclusion: This study highlighted that UI affect women of all age groups. It is an important point that majority of them do not seek treatment. Thus, there is a pressing need to raise awareness among public and healthcare providers. Additional studies are therefore indicated. |

Is Traditional Chinese Medicine (TCM) – Guilin Watermelon Frost (Insufflations) Better for Wound Healing and More Effective Compared to the Current Hydrocolloid Wafer Dressing Used for Treating Pressure Sores? |
Aims: To test the efficacy and evaluate the cost effectiveness of Sanjin Watermelon Frost (Insufflations) with conventional hydrocolloid wafer dressing by comparing the wound healing rate for stage II and stage III pressure sores.
Methods: Twenty seven subjects of stage II (n=19) and stage III (n=8) pressure were randomly allocated to be treated with either Sanjin Watermelon Frost (Insufflations) (n=14) or hydrocolloid wafer dressing (n=13). All subjects were assessed by the same investigator for a) wound healing assessment using the Pressure Sore Status Tool (PSST) on the day of recruitment and every three day thereafter; b) weekly photography and c) base line nutritional assessment using a validated Nutritional Status Assessment Tool. A data collector was assigned to follow up with the dressing until the pressure sore healed but not exceeding15 days.
Results: Six pressure sores healed within 3 to 15 days. Two pressure sores healed on Day 3 and Day 15 in the treatment group and four pressure sores healed on Day 12 in the control group. Five subjects in each group were terminated prematurely due to deterioration in medical conditions. Results in this study showed that statistically there were no significant difference (p=0.532) in the wound healing rate between the treatment and control group using the longitudinal data analysis technique. The raw material cost per dressing in the treatment group is cheaper than control groups by 25.3%. However, with the inclusion of manpower cost and assumption that all pressure sores were treated for 15 days, hydrocolloid wafer dressing is more cost effective than treatment group by 35%.
Conclusions: On conclusion, there could be a cost justification and clinical benefit of using SanJin Watermelon Frost (Insufflations) in the management of pressure sores as it increases the opportunity of achieving wound healing. |

Randomised clinical trial comparing the use of Mepitel with conventional paraffin gauze as dressing material for patients with skin graft. |
Introduction: Dressing changes are often time consuming for nurses. Care must be taken to minimise disturbance to the vulnerable new skin grafts. Patients experienced varying degrees of pain, especially when dressings adhere to the grafted site. Mepitel® has a non-adherent property due to the silicone coating thus offering an alternative dressing material to the conventional paraffin gauze (Jelonet) dressing, which at times adheres to grafted skin.
Aims: To evaluate the safety and efficacy of Mepitel® versus conventional Paraffin gauze dressings with respect to (i) ease of dressing change (ii) patient comfort (iii) grafts infection and (iv) grafts take.
Methods: A randomised controlled trial was conducted on forty patients (age: 16 – 96 years) who underwent skin grafts for wound coverage in the Department of Plastic Surgery, Singapore General Hospital . There was equal representation in terms of gender and race in the two groups of patients. Data on ease of removal of dressing material, degree of pain experienced at each dressing change, graft infection and graft takes were collected.
Results: The study showed a significant difference in terms of ease of dressing removal (p = 0.00), pain score during dressing changes (p = 0.001), graft infection (p = 0.037) between the two dressing group. The percentage of skin graft take (p = 0.165) was consistent with other studies conducted on the two types of dressing materials.
Conclusions: Although Mepitel® costs more than Paraffin gauze dressing, the potential benefits outweighed Paraffin gauze in terms of patient comfort and satisfaction, reduction on time spent on dressing changes, graft infection and graft take. |

Perineal Cold Pad Vs Oral Analgesics in the Relief of Post-Partum Perineal Wound Pain |
The primary cause of perineal pain is from bruising of the perinuem due to episiotomy or tearing of the perineum during vaginal delivery. Perineal pain in early postnatal period had been reported to be one of the commonest causes of maternal morbidity; it also impairs a mother's ability and willingness to care for the newborn. Currently, patient is prescribed with the standard regime of oral analgesic for pain relief. This study was undertaken to compare the effectiveness of perineal cold gel pad vs standard regime of oral analgesics in post partum perineal pain relief.Two hundred and twenty patients who delivered their first or second baby by normal vaginal delivery or assisted vaginal delivery with an episiotomy done were selected and randomized into the control and experimental group. The control groups were given standard regime of oral analgesics for pain relief and the experimental groups were given one packet of 4 perineal cold gel pads post delivery. Assessment and pain score rating was done by nurses.Pain score for the cold gel group was statistically significant, P<.000 after 6 hours of pain relief. 97.3% of patients were satisfied with the cold gel pad and its effectiveness in absorbing the lochia as well as their ability to ambulate well and breastfeed their babies. This study showed that the use of cold gel pad, a non-invasive method was as effective in the reduction of perineal pain. |

Objective: The study used qualitative method to determine the health seeking behavior of HBV carriers. Understanding such behavior will enable the attending health professionals to optimize their care and promote regular disease surveillance.
Method: Data were collected from 39 HBV carriers from primary and secondary healthcare centers and different demographic profiles in 8 respective focus group discussions (FGD). A nurse conducted the FGDs using a semi-structured guideline. The qualitative data were analyzed using standard content analysis technique.
Results: There was evidence of doctor hopping amongst the HBV carriers in seeking the follow up of their disease. Cost of review and investigations and preference for specialists' care appeared to be determinants of the sites of disease monitoring. A significant proportion of the carriers had also tried alternate therapy, took liver supplements but most found them to be ineffective. Most carriers had indeed adopted a healthier lifestyle with regular exercise, smoking cessation and alcohol abstinence.
Conclusion: Many HBV carriers' inadequate understanding of the disease resulted in indifferent or
inappropriate health seeking behavior towards their disease management. Educating and ensuring
that these carriers understand the natural history of the disease, the rationale for disease
surveillance and the current pharmaceutical therapy for chronic HBV infection would be the three
key areas in the management of the disease in primary care. |

Exploratory study on the attitudes of renal nurses in chronic dialysis setting towards palliative care in end stage renal disease |
Nurses and other health care professionals play an important role in palliative care for end stage renal disease patients. There are no reported studies in Singapore on this area. To have a better understanding of the local nurses' views on palliative care for renal patients, this study explore the attitudes of renal nurses working in chronic dialysis settings towards palliative care, and how comfortable nurses are in discussing palliative care with end stage renal disease patients and their families who have opted to discontinue dialysis.
This descriptive study was conducted using a sampling of 100 registered renal nurses working in an acute restructured hospital. “Attitude towards death” survey was used to assess renal nurses' view on palliative care. The results of this study indicated that majority of the respondents felt something can be done to help individuals achieve a sense of peace at the end of life. This research result also showed that almost half of the respondents felt that they were comfortable talking to families about death. The respondents felt that the greater degree of family involvement, education and training in pain control and palliative care team involvement were important aspects to improve the end of life care for ESRD patient. |
